Sa nakalipas na linggo, maraming tumawag at nag-email sa akin hinggil sa Funeral Benefit. Pinili ko ang ilan sa mga ito upang sagutin mismo sa aking column ngayong linggo:

Good day Sir, tungkol po sa burial benefit, iba po ba ito sa death benefit? Paano ito makukuha ito sa SSS? – Reynan Santiago, Aringay, La Union

Good morning SSS, may nakapagsabi sa akin na may makukuha pa daw kaming pera from SSS kahit may nakuha na kaming lump sum noong namatay ang tatay ko noong 2016. Ano kaya ‘yun at paano ito makuha? Thanks po – Madelyn Osias, Baguio City

Hello SSS, same requirements ba ang funeral benefit sa death benefit? I believe these two benefits are different. And should we file it at the same time? Thank you – Francesca, Baguio City

Hi! My mom died in Lebanon but she is an active SSS member. Being an OFW, I pay her contributions yearly. Are we still entitled for Burial Benefit? How can we pursue the application? Una sa lahat, nais kong ipaliwanag na magkaiba ang Funeral sa Death Benefit. Ang Funeral Benefit o Benepisyo sa Pagpapalibing ay cash benefit na ibinibigay sa sinumang gumastos sa pagpapalibing ng namatay na miyembro. Hindi kinakailangang kamag-anak ng namatay na miyembro o SSS member ang mismong claimant. Basta’t kung sino ang nagbayad sa pagpapalibing at sa kanya nakapangalan ang resibo mula sa punenarya, siya ang maaaring tumanggap ng benepisyong ito.

Simula, Agosto 2015, ang halagang matatanggap ay mula sa minimum na P20,000 hanggang sa maximum na P40,000, depende ito sa credited years of service (CYS) o bilang ng mga taon ng paghuhulog ng kontribusyon sa SSS ng miyembro at ang kanyang average monthly salary credit (MSC) na siyang batayan ng komputasyon ng nasabing benepisyo.

Para ma-qualify, kinakailangang nakapaghulog ang miyembro ng isang buwang hulog bago ang semestre ng kanyang pagkamatay.

Para makapag-claim, kailangan mag-fill out ang claimant ng SSS Funeral Benefit Claim Form na makukuha sa branches o kaya naman ay maaaring i-download sa SSS website, www.sss.gov.ph.

Kailangan ilakip sa application form ang Official Receipt (OR) mula sa punerarya kung saan nakasulat ang pangalan ng claimant at ng namatay na miyembro. Kung sakaling nawala ang resibo o kailangan ipasa sa ibang ahensya, ipakita ang alinman sa mga sumusunod: certified true copy ng OR, o certification mula sa funeral parlor/ memorial service kung saan nakalagay ang mga detalye ng mga binayaran sa pagpapalibing ng namatay na miyembro.

Isama rin sa Funeral Benefit Application ang mga dokumentong gaya ng ID na nagpapatunay ng pagkakakilanlan ng namatay na miyembro, ID na nagpapatunay sa pagkakakilanlan ng claimant, Letter of Authority o Special Power of Attorney kung ang mag-file ng claim kinatawan ng claimant, original/duplicate/certified true copy/certified photocopy ng Death Certificate ng namatay na miyembro, Death Certificate mula sa Vital Statistics Office/County ng host country o ang katumbas nito kung ang miyembro ay namatay at nailibing o na-cremate sa ibang bansa. Importanteng may opisyal na pagsasalin sa wikang Ingles mula sa Department of Foreign Affairs (DFA) o Embahada ng Pilipinas kung ang Death Certificate ay hindi nakasulat sa Ingles.

Kung may memorial o insurance plans ang miyembro, mag-submit ng Certification of Availment mula sa memorial/insurance life plan company. Ito ay para sa mga piling beneficiary lamang. Subalit, kung ang memorial/insurance plan ay nasa pangalan ng claimant, kailangan pa ring mag-submit ng Certification of Availment na kung saan nakasulat ang pangalan ng namatay na miyembro, claimant, at petsa ng pagkamatay. Kung sakaling hindi nakasulat sa Certification of Availment ang pangalan ng namatay na miyembro ay, mag-submit ng notarized Affidavit mula sa plan holder bilang patunay na pinapayagan ang namatay na miyembro na gamitin ang kanyang memorial/insurance plan.

Kapag ang memorial/insurance plan ay wala naman sa pangalan ng namatay na miyembro at claimant, maaaring ipasa Certification of Availment at alinman sa mga sumusunod tulad ng: Transfer of Certificate, Deed of Sale, at Deed of Donation o Affidavit of Sale mula sa plan holder.

Kapag nai-file na ang Funeral Benefit ay saka lamang maaaring mag-submit ng Death Benefit claim. Ihanda ang mga sumusunod na requirements gaya ng SSS Death Benefit Application Form na maaaring i-download sa sss.gov.ph, Filer’s Affidavit, Member’s/Claimant’s Photo, Death Certificate na certified true copy mula sa Philippine Statistics Authority (PSA), Marriage Contract na certified true copy mula din sa PSA, single savings account passbook/ATM card, at dalawang valid IDs na may litrato at pirma ng filer/claimant.

Simula June 22, 2020, ang SSS Funeral Benefit ay ginawa ng checkless disbursement kung saan binabayaran na sa pamamagitan ng bank account ng filer/claimant o sa SSS cash remittance partners tulad ng Mlhuiller. Kinakailangan lamang na rehistrado ang kanilang mga bank accounts sa Bank Enrollment Module ng SSS. Simula Agosto 2020 naman, ang mga aplikasyon ay maaari ng gawin via online sa My.SSS Facility sa SSS website, www.sss.gov.ph. Para sa mga karagdagang impormasyon, maaaring bisitahin ang opisyal na Facebook page ng SSS, ang Philippine Social Security System.
